What you need to help find immunization records:

  • Baby record books or scrapbooks with mementos
  • Childhood doctors' names and phone numbers
  • City and county phone numbers for vital statistics
  • Social Security or other ID with your childhood name
  • Fireproof storage for safe-keeping

 

  1. Ask your mother or whomever was your guardian when you were a child. She may have a file of immunization records tucked away.
  2. Check any baby books or scrapbooks that were kept with your baby records. Many people keep mementos for their children for reference in their later years.
  3. Contact the childhood doctors who may have administered immunizations for you or your family. Many medical offices keep patient records on file dating back decades. Allow enough time for them to search their old files.
  4. Check with the city or county where you lived as a child or where you received immunizations. Some cities' or counties' departments of vital statistics may keep medical and immunization records on file and be able to access these for you.
  5. Keep a copy of all immunization records in a safe, fireproof container once you have located them. Consider buying a small safe for stashing important papers. These are inexpensive and are a great way to let your family members know where any important papers and immunization records are located.

 

Although it may be a challenge, in most cases, a diligent search will reward you with copies of all immunization records for yourself and your family members.
